At this time I currently accept the following insurances:
United (Optum, Oscar)
For those with another type of insurance I can provide you with a receipt (Superbill) that you can submit to your insurance company for out of network reimbursement.
Payment is due at the time services are received. I accept cash, check, and credit cards/HSA cards. Please note that payment is due at the beginning of each session if you do not have a credit card on file.
I require a 24 hour notice for cancellation of an appointment. If you cancel or miss a scheduled appointment and have not given me 24 hours notice, or show up 15 minutes or later to an appointment, the appointment will be considered a no show and you will be charged a $100 no show/late cancel fee. *Those with Medicaid are not charged for missed sessions, but will be referred out after 3 missed sessions in a 12 month period.*
